Billing and Insurance
At Jackson Siegelbaum, we work hard to keep our costs low while still providing you with the highest quality gastroenterology care. We are confident you’ll find the fees for our services are comparable to those charged by similar specialists in the area. Our fees are based on the services provided to you and a list of service fees is available upon request.
We participate in many insurance networks to provide as much coverage as possible for the services you need. However, insurance coverage varies by person and each patient is handled individually. So, it is important that you understand what your plan covers and how much of the payment you are responsible for at the time of service.
For example, most plans will cover outpatient testing. Some Blue Shield plans do not cover consultations and office visits. Many HMO plans cover authorized office visits, but request that a co-payment be paid at the time of the service. And patients covered by commercial insurance plans are responsible for the full amount of the charges.
It is important to bring the following insurance forms to your appointment:
- HMO authorization form
- Precertification form
The staff at Jackson Siegelbaum will submit all insurance forms on your behalf whenever possible.
Do you have major medical insurance? Please note that major medical claims must be submitted by the patient because the insurer makes the payment directly to the patient.
